I saw an interesting entry on why TIVO hasn't taken off faster and several claims about implementation/acquisition costs but I think they all missed the main point: people much prefer one-time costs than subscription services. I tried to sell the idea of the TIVO to my mom but she just didn't think the value add of commercial skipping and the machine knowing what was on when was worth the $10-15 dollars a month that TIVO charges. Otherwise, I think she (and most people who are moderately savvy about technology) would have been on the TIVO bandwagon long ago.

Some people may be more than willing to pay $5-20 a month if there is little to no up front cost. They may think they can make better use of their money in the long term, whereas some people may not care, and shell out the larger amount from the get-go.
